Documentary Treasures Submission Form Instructions

Please type all information in the response boxes of the Submission Form before you print it out from your Computer. If you do not know how to print your completed Submission Form, click on the command "Activate" shown on the bottom of the Submission Form and then click on your "Print" command.

If you want to submit a subsequent Submission Form, replace previous information with new data where it is necessary and "Submit" and "Print" again.

Fee for each documentary archived: $25.00

Fees are subject to change.

Payment Submittal

Make Annual Fee payment via check payable to: Documentary Treasures, Inc. Submit a separate check for each Submission Form submitted. Please note the name of the documentary to be entered on the memo portion of the check.

Return your check attached to your Submission Form(s) to:

Incomplete Submission Forms and / or failure to submit the required payment will delay / nullify the entry of your data into the Documentary Treasures web site.

Documentary Treasures, Inc. does not deal with orders for documentaries. Any selling/buying/lending transactions are between the documentary owner and the purchaser.

Documentary Treasures, Inc. does not endorse or warrant any documentary listed in our database.

Our intent is to create as comprehensive a database as possible, without regard to origin, quality or content, and we do not intend to evaluate any documentary in any way. However, Documentary Treasures, Inc. reserves the right to remove any documentary from the database as required by law.
